Attendance / Preparedness	Excellent Brought all needed materials (including book, paper, and a pencil) to class every day. Arrived to class on time every day.	Good Materials brought to class 4 of the 5 school days. Arrived on time at least 4 of the 5 school days.	Fair Forgot materials more than once, but not every day. Arrived at class on time most days.	Poor Without needed materials every day OR arrived late frequently.
Behavior/Effort	Excellent I was on task nearly all of the time. I gave 100% effort and tried as hard as I could. The teacher did not have to speak to me at all about my behavior.	Good I was off-task a few times. I put forth strong effort, but could have worked harder at times. The teacher had to speak with me only once about my behavior.	Fair I was off task quite a bit. I didn't try very hard at all or was slow to start assignments in class. The teacher had to speak with me about my behavior more than twice this week.	Poor I was off take most of the time. I refused to do assignments. The teacher had to speak with me three (3) or more times this week.
Homework	Excellent All homework brought in on time, completed, accurate, and with all work shown.	Good Homework was brought in on time at least 3 days this week. It was done accurately and work was shown.	Fair Two (2) or more homework assignments were missing or homework was turned in that was significantly incomplete.	Poor No homework was completed this week.
Classwork	Excellent All classwork was completed on time and accurately. All work was shown.	Good Classwork was completed on time and is mostly accurate or is all accurate, but mostly complete.	Fair Very little classwork was done; or classwork was incomplete.	Poor No classwork was done.
