Note Quantity	Exceptional All notes are accounted for.	Good Almost 100% of notes are accounted for.	Fair There are 2/3 or less of the notes that are supposed to be there.	Poor There are 1/3 or less of the notes that are supposed to be there.
Note Content	Exceptional Notes cover main points of content.	Good Notes cover most of the content.	Fair Notes cover some main points of content.	Poor Notes have little or no main points of the content.
Note Organization	Exceptional Notes are easily read and organized by date.	Good Notes are easily read and there is some organization of the notes	Fair Notes are easily read but there is no system of organization.	Poor Notes are messy and cannot be read.
