Middle School Research - Survey Market Research - Survey This rubric is to assess student use of Google Forms to create a meaningful and useful survey to collect data to be used in developing a plan to improve ANS facilities and/or procedures. Market Research Survey
	Poor 5 pts	Fair 10 pts	Good 15 pts	Excellent 20 pts
Number of questions	Poor Student includes 4-5 questions on survey.	Fair Student includes 6-8 questions on survey.	Good Student includes 9-11 questions on survey.	Excellent Student includes 12-14 questions on survey.
Types and quality of questions	Poor Student uses only one type of question format (ex: multiple choice/one answer) and questions gather very basic data.	Fair Students uses at least two types of question formats (ex: multiple choice and short answer) and questions gather basic data.	Good Students uses at least two types of question formats (ex: multiple choice and short answer) and questions gather opinions and suggestions as well as basic data.	Excellent Students use more than two types of question formats (ex: multiple choice/ one answer, multiple choice/multiple answer, essay, etc) and questions gather in-depth information from respondents.
Number of responses	Poor Student gathers 10-20 responses.	Fair Student gathers 21-35 responses.	Good Student gets 36-50 or more responses.	Excellent Student gets 51 or more responses.
Analysis of data	Poor Student provided list of results by category.	Fair Students provided list and tally of results of changes to facilities and/or procedures.	Good Student provided list and tally of results and general recommendations for improvement to facilities and/or procedures. Written Report	Poor Student provides report of one to two sentences per section. 1-2 pages in length.
