Goal Setting Project Rubric Goal Setting Project Rubric Students will demonstrate the ability to make decisions and set goals to enhance health. Students will identify a personal health goal and outline the process of achieving the goal using the SMART goal acronym. Students will define the goal,step by step, and keep track of their progress using the SMART outline. Students will reflect on whether they achieved the goal or how they will adjust it to keep working on it through reflection questions. Goal Setting
	Poor 1 pts	Fair 2 pts	Good 3 pts	Excellent 4 pts
Goal Setting & Process	Poor Student did not provide a clearly defined goal using the SMART goal language to be achieved and/or showed little or signs of understanding the process of completing a gaol	Fair Student provided a goal to be achieved and/or showed some understanding for the goal setting process.	Good Student stated a clearly defined goal using the SMART goal language and showed complete understanding for the goal setting process.	Excellent Student went above and beyond the requirements for goal setting and in their understanding of the goal setting process.
Check-ins	Poor Student only provided one or less progress check-ins	Fair Student provided most of their progress check-ins but a clear progress to their goal was not achieved.	Good Student provided check-ins and clear progress was made	Excellent Student went above and beyond the amount of check-ins required and a clear path to success could be drawn
Reflection/Knowledge	Poor Student did demonstrate knowledge or provide a reflection on their goal and whether it was achieved or not.	Fair Student somewhat demonstrated knowledge and provided a reflection but 4 or less questions answered in reflection.	Good Student demonstrated knowledge(as required) through reflection by answering all questions in a clear and concise method	Excellent Student demonstrates full knowledge (more than required) by answering all class questions with explanations and elaboration.
Complete & Timely	Poor Not turned in at all	Fair Turned in assignments mostly on time. Meets a few of the requirements.	Good Turned in assignments on time and met all requirements	Excellent Student completed all assignments on time and went above and beyond what was required.
