Conduct 25 %	Level 1 -seldom displays positive behavior	Level 2 -usually displays positive behavior, but will often become use the equipment inappropriately.	Level 3 -regularly displays positive behavior and is rarely seen using the equipment inapproriately	Level 4 -always displays positive behavior and always uses equipment appropriately.
Physical Participation 50 %	Level 1 -infrequently participates in a variety of physical activities<BR> <BR> -experiences difficulty participating as an individual or in a group setting	Level 2 -sometimes participates in a variety of physical activities<BR> <BR> -sometimes participates positively as an individual or in a group setting	Level 3 -regularily participates in a variety of physical activities<BR> <BR> -frequently participates positively as an individual and in a group setting	Level 4 -always or almost always promotes active participation in all physical activities<BR> <BR> -always participates positively as an individual and in a group setting
