	Exceptional 5 pts	Satisfactory 4 pts	Unacceptable 0 pts
Accuracy 25 % Each encounter provides accurate CPT and ICD-10 codes.	Exceptional The student provides consistent, accurate CPT and ICD codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information.	Satisfactory The student provides, for the most part, consistent, accurate CPT and ICD codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information. There are occasional errors.	Unacceptable The student has multiple errors in CPT and/or ICD codes for the type of visit, PMH, procedures performed, diagnostic testing, and any other pertinent information.
Completion 25 % Each encounter provides a complete picture of all areas addressed during the visit (i.e. social issues, procedures/competencies).	Exceptional The student provides complete case logs.	Satisfactory The student, for the most part, provides complete case logs.	Unacceptable The student has multiple incomplete case logs.
Professionalism 50 % Students demonstrate accountability, timeliness, interpersonal relationships, and commitment to their educational experience.	Exceptional The student demonstrates professionalism by: being accountable to the program, as well as the clinical site, being timely in submissions, absence requests, & responses to emails, being professional in their relationships, and being committed to their education and clinical experience.	Satisfactory The student, for the most part, demonstrates professionalism but could use improvement in a certain aspect.	Unacceptable The student has one or more area of concern that need significant improvement in regards to professionalism.
