Before Catheter Insertion

Check Doctor's orders, gather equipment, provides patient with privacy, explains the procedure to patient and explains positioning to the patient, identifies the correct patient. Use self-correction to prevent the commission of errors.

Performs all initial steps without difficulty. The nurse stops to ask questions if unsure of the next step prior to completing the action (to the evaluator).
Does Not Meet

The nurse does not provide safety, privacy, or explanation to the patient. The nurse does not ask questions to ensure the next steps of patient care. The nurse is disrespectful to the patient.
Sterile Field setup

Washes hands, uses clean gloves to open kit, prepares the kit as a sterile field and applies sterile gloves. Lubricates the end of the catheter. Utilize principles of medical and surgical asepsis in the performance of all nursing skills.

The nurse washes hands prior to beginning. The nurse maintains a clean technique followed by sterile technique. The nurse asks questions or asks for assistance if unsure of the next step (to the evaluator).
Does Not Meet

Nurse does not wash hands prior to beginning. Nurse does not correctly identify or set up a sterile field or breaks sterility. Nurse experiences difficulty and does not ask for assistance.
Cleansing the Patient

The nurse uses the non-dominant hand to clean the peri-urethral area and uses the dominant hand to utilize forceps and cleansing solution to clean using anterior to posterior, inner to outer, and using one swipe per swab, discarding away from the sterile field. Utilize correct body mechanics during the performance of all nursing skills.

Nurse establishes to the evaluator which hand is dominant and sterile and which hand is non-dominant and the "dirty" hand. The nurse cleanses the patient properly and discards the dirty swab after one swipe. The nurse asks for assistance if needed.

Inserting the Catheter

Picks up catheter with sterile hand. Locates meatus and Inserts catheter while maintaining sterility. Inflates balloon with full amount of saline in syringe. Places attached stat-lock to patient leg and connects urinary catheter tubing.

a. uses sterile hand to handle catheter
b. Locates meatus
c. Inserts catheter and maintains sterility
d. Inflates balloon with full amount of normal saline in provided syringe.
e. Uses skin prep and attaches stat-lock and places catheter in stat-lock.
f. Maintains respect for patient.
g. Asks evaluator for assistance if needed.
Does Not Meet

The nurse fails to maintain sterility during the insertion of the catheter. Does not meet or maintain privacy for patients or respect for patients' needs.
After Insertion

Nurse describes evaluation of catheter function, urine amount and quality of urine and if specimen is needed.
Removes gloves and discards trash. Washes hands with soap and water. Utilize accurate documentation and appropriate patient education related to performing nursing skills.

Meets

Nurse states to the evaluator:
a. catheter is patent or needs to be reinserted (nurse decides)
b. amount of urine output and quality
c. that gloves are removed and hands are washed using soap and water
d. explains how insertion will be documented
Does Not Meet

Nurse does not announce urine output, glove removal and hand-washing, does not explain documentation.
Re-Evaluation

Nurse assess indwelling catheter shortly after to ensure patency, patient comfort or pain, urine output and any other additional orders.

Meets

The nurse explains to return to the patient and re-assess the urinary catheter, ensure patency, level of comfort for the patient, continued urine output or not and foresee possible other orders.
Does Not Meet

The nurse does not explain or forgets to return to the patient and re-assess the urinary catheter, the nurse does not ensure patency, level of comfort for the patient, or continued urine output or not and foresee possible other orders.
