	EXCELLENT 4 pts	GOOD 3 pts	FAIR 2 pts	POOR 1 pts
Response	EXCELLENT The response is very specific to the task; all prompts are addressed and clearly stated and supported.	GOOD The response refers to the task; major points are made, but only somewhat supported.	FAIR The response refers to an unspecified task; answer does not clearly respond.	POOR The response does not specify the task; confusion on connection to prompt and answer.
Information	EXCELLENT Information is accurate and response shows penetrating insight. The task is referred to in the answer.	GOOD Information is accurate. A logical conclusion or an opinion is offered.	FAIR Information provided is generally accurate but no insight is offered.	POOR Information may be missing or inaccurate. No insight is shared.
Format	EXCELLENT Format is as outlined and instructed with date, form, prompt and answer complete.	GOOD Little deviation from assigned format; over or under length.	FAIR Several deviations from format; paragrapsh are inadequate; does not follow primary directions.	POOR Little or no resemblance to format.
Accuracy	EXCELLENT Conclusions and/or opinions are logical. Overall impression: complete and satisfactory.	GOOD The answer is lengthy rather than concise / or too short and not enough info.
