This rubric is used to evaluate a nursing student's portfolio to determine if its contents represent the achievements of the student in realizing the program outcomes established for the Registered Nursing degree.

Journal entries from final preceptorship include:

No reflections are recorded in the portfolio.

Portfolio demonstrates 1-3 reflections,
Good

Portfolio includes 4-8 general reflections for each benchmark, including some supporting details and examples.
Excellent

Portfolio demonstrates an in-depth reflection on the selected topics with supporting details and/or examples are clearly identified.
Growth

Growth is demonstrated by reviewing the care plans and concept maps the student submitted over the course of the nursing program. the student's work should demonstrate an expanded understanding of nursing concepts from the first semester to the last semester.

Poor

No growth made, no effort displayed in reflections. Concept maps and care plan remain rudimentary. No signs of developed clinical judgement.
Fair

Some evidence of clinical judgment is emerging, Concept maps and care plans demonstrate some inclusiveness,
Good

Growth in the student's ability to make sound clinical judgement decisions to keep patients safe.
Excellent

Student demonstrates a clear and high level of clinical judgement in concept maps and care plans excellent growth displayed.
Effort

Effort is evidenced by how the portfolio is organized. Are the files in folders? Are the folders files are in folders, if there are folders for the amount

Poor

No effort was made. Files are uploaded into Portfolium but no organization is seen.
Fair

There are a few files with appropriate labeling.
Good

Several or all files made, but they are not labeled according to the instructions.
Excellent

Excellent Effort. Files are made and appropriately labeled.
Maximum Points: 20
Must earn at least 16 points to obtain a passing grade.
