Fair The form/survey has 6 or 7 questions with rankings and at least one open-ended question. The form/survey has one or more spelling/grammar errors.	Poor The form/survey has fewer than 6 questions with or without rankings and no open-ended questions. The form/survey has more than one spelling/grammar errors.
Evaluation Form/Survey Results	Good Results are tabulated and presented in a table or graph that is easy to read. Results include all questions and results pertaining to students' responses.	Fair Results are not clearly presented. Results do not include the questions with corresponding responses.	Poor The results are missing from the reflection.
Evaluation Form/Survey Reflection	Good The critique/reflection is thoughtful and includes at least 8 specific reflections about the results.	Fair The critique/reflection is somewhat thoughtful and includes fewer than 8 specific reflections about the results.	Poor The critique/reflection is not thoughtful and includes fewer than 6 specific reflections about the results.
