ORGANIZATION	EXCEEDS ALL Notes follow an approved format.	MEETS MOST notes follow a format.	UNSATISFACTORY Less than 1/2 of notes follow a format.
LABELING	EXCEEDS ALL sections are labeled properly.	MEETS 75%- 99% of sections are labeled properly.	UNSATISFACTORY <75% of sections are labeled properly.
COMPLETNESS	EXCEEDS Notes contain all requirements for each sections.	MEETS Notes are missing a no more than 1-2 requirements for each section.	UNSATISFACTORY Notes are missing 3 or more requirements on average for sections, or Missing a whole section.
