Introduction	Poor All but three of the (4) criteria are met.	Fair All but two of the (4) criteria are met.	Good All but one of the (4) criteria are met.	Excellent 1) Arrives on time or early. 2) Wears a suit and is well groomed. 3) Offers hand and introduces self. 4) Comes off friendly and seems like someone I would be happy to see in a medical setting.
Mental Preparation	Poor Answers 50% of all questions with all (4) of the criteria being met. -or- Answers all questions with (1) of the criteria being met.	Fair Answers 70% of all questions with all (4) of the criteria being met. -or- Answers all questions with (2) criteria being met.	Good Answers 80% of all questions with all (4) of the criteria being met. -or- Answers all questions with (3) criteria being met.	Excellent 1) Answers all questions immediately. 2) Answers all questions confidently. 3) Answers all questions thoroughly yet concisely. 4) Answers all questions with enthusiasm and thoughtfulness.
Personality	Poor All but three of the (4) criteria are met.	Fair All but two of the (4) criteria are met.	Good All but one of the (4) criteria are met.	Excellent 1) Personality shows throughout the interview. 2) The interviewee is likable. 3) The interviewee will be memorable. 4) The interviewee shows a passion/enthusiasm to be at the interview.
