Excellent (A)
Provides a thorough documentation of relevant questions needed in the history inclusive of duration, onset, severity with a justification for asking the questions and support the differential diagnoses.
Good (B)
Provides limited documentation of questions to support the differential diagnoses but includes duration, onset and severity
Satifactory (C)
Provides some questions that are not relevant and unconvincing in significance but includes at least duration, onset or severity
Weak (D)
