With average guidance and prompting can answer questions and map out solutions to problems.
Repeat work is average. Meets expectations
Above Average

Knowledge of concepts is evident and critical thinking is regularly observed. Demonstrates sound knowledge base. With little prompting and some initial guidance, able to spot problems, answer questions, and map out course of action. Exceeds expectations
Exceptional

Self directed. Regularly makes connection between results and conditions, technical issues, problems, descrepancies. Demonstrates sound knowledge that is regularly above expectations.

Needs Improvement

Slow to adjust to changes and new material. Requires major review of processes previously covered from preceptor greater than 75% of the time. Does not use procedure manuals or notes to review processes. Shows minimal confidence in ability to perform.
Average

Adjusts to changes and new material. Uses procedures and notes to review work prior to beginning tasks. Performs tasks as assigned with minimal questions on basic processes. Shows confidence in ability to perform tasks with appropriate guidance. Meets expectations for this level.
Above Average

Exceeds expectations in performing tasks. Review procedures and notes and can begin tasks with minimal prompting from preceptor as appropriate for student level. Readily grasps new material and changes.
Shows confidence in ability to perform tasks previously completed.
Exceptional

Above and beyond expectations. Able to repeat tasks with use of notes, procedures,and rare prompting as appropriate. Grasps concepts, new material, changes, and assimilates them with increasing confidence.

Lack of confidence in own judgment. Cannot proceed without complete instructions from preceptor- May pose risk to patient/self/instructor/staff without intervention.

Judgment is sound. can make assesments of situations and does not pose risks to patient/self/instructor/staff. Ask questions when in doubt rather than injure patient/self/instructor/staff. Confident in decision making ability at appropriate level for student. Meets expectations.
Above Average

Makes sound judgements and suggestions. Offers solutions to issues. Understands when to ask questions. Confident in decision making ability. Exceeds expectations in judgement.
Exceptional

Above and beyond expectations of judgement for student level. Understands consequences of actions. Makes suggestions and sound decisions and asks appropriate questions when in doubt. Shows exceptional maturity and confidence.
