Response	Poor Unorganized Slow response and hesitation during setup. Confidence lacking. Setip locations poor	Fair Response timely, but setup at incident could be improved.	Good Acceptable response and initial deployment of personnel
On Scene Management	Poor Did not check for injured or deaths, disregarded personal safety and that of other emergency workers. Disorganized, no command post, no delegation, no resources	Fair Slight improvement . Resource management only fair	Good perimeters, command post and delegation was appropriate. Additional resources contacted and deployed. Took care of personnel and their needs
Overall Performance	Poor Safety, organization and management of crisis not acceptable, incident actually became more of a problem that the original report personnel possibly injured	Fair Still needs major improvement in most areas.
