Proposal for Your Personal Change Project 
This proposal will include the statement of your personal goal, what you will measure, 5-7 days of baseline measures, time frame for completion of your project(e.g. how long will you measure and work on your goals), the theory you are using to reach your goals, and the name of our coach.
