The Five Senses
	Poor 1 pts	Fair 2 pts	Good 3 pts	Excellent 4 pts
Name/Describe the 5 senses	Poor Can't name or describe the five senses.	Fair Can name some of the senses, but not describe them.	Good Can name most of the senses and describe them.	Excellent Can name and describe all five senses after the lessons being taught.
Can identify things using the 5 senses	Poor Can't identify things using the five senses.	Fair Can identify things, but does not understand how they are able to identify it.	Good Can identify things and understand.	Excellent Can identify things and understand what they are identifying as well as understanding which body part is being used in the process.
Understand the body parts being used	Poor Can't identify any of the body parts being used for the five senses.	Fair Can only identify 2-3 of the body parts used for the five senses.	Good Understands the five senses and can match 4 out of 5 body parts to each sense.	Excellent Can identify the body parts that are used for the five senses. The mouth is used for taste, eyes for sight, ears for hearing, hands for touch, and nose for smell.
Can use the 5 senses everyday	Poor Can't understand the five senses and does not understand when it is being used in their everyday life.	Fair Can understand some of the five senses that they use throughout the day.	Good Can understand four out of the five senses that they use throughout the day.	Excellent Can understand when they are using their five senses everyday from tasting, touching, smelling, listening/hearing, and seeing things throughout the day.
