Student performed leak test properly, proper size tubes and bag was placed on the machine, levels of Isoflurane were checked, levels of oxygen were checked.

Student asked for help with the leak test, needed prompting for the proper tubes and bags.

Leak test was not performed, fluid levels not checked and/or oxygen level not checked. Correct tubes AND reservoir bag were not attached

Calculations were correctly performed and drugs were drawn up correctly....correct size syringes were used.

Student chose the correct catheter size, had all supplies ready, placed the catheter confidently, flushed it successfully and taped it in securely.

Patient was transferred to the surgical suite without contamination

Student monitored patient both physically and with the help of mechanical monitors, did not need prompting to correct anesthetic related incidents or change isoflurane levels, acknowledged the DVM during surgery, filled out the anesthesia form completely and correctly.

Student monitored the patient prompting from the instructor, only missed 1 recording on the anesthesia form, acknowledged the DVM during the surgery start/finish times, did not rely on the mechanical monitors to maintain anesthesia, changed isoflurane with prompting from instructor

Student did not physically monitor the patient but relied on the mechanical monitors, did not take initiative when the patient needed isoflurane levels changed, failed to record properly on the anesthesia form, missed more than 2 entries on the anesthesia form, did not acknowledge DVM on surgery start/finish times

Student charted all medications correctly with mg's and ml's, all drugs and procedures were spelled correctly, times were recorded correctly, all information was recorded in Avimark correctly, all paperwork was filed correctly.
