Relationship with Others	Excellent Exemplary working relationship with others.	Above Average Works well with others.	Average Gets along satisfactorily with others.	Below Average Has some difficulty working with others.	Unacceptable Works very poorly with others.
Attitude / Application to Work	Excellent Outstanding and positive attitude with enthusiasm.	Above Average Very motivated and industrious.	Average Average in diligence and motivation.	Below Average Somewhat indifferent in attitude.	Unacceptable Shows lack of interest.
Initiative/Independence	Excellent Independently innovates or finds and develops solutions. Uses practical and logical thought to achieve solutions.	Above Average Takes initiatives on 80-90% of the tasks.	Average Requires some directions or does not take initiative to complete tasks independently 50% of the time.	Below Average Does not initiate tasks or development of solutions on 80% of tasks	Unacceptable Never initiates any tasks or the creation or discovery of solutions.
Quality of Work	Excellent Independently completes high quality work.	Above Average Needs one prompt per shift to complete quality work.
