Wound Healing Discuss appearance of surrounding tissue	Poor The wound and tissues surrounding wound are briefly described or not described at all.	Fair The wound appearance is partially described but missing some specifics regarding surrounding tissue.	Good The Tissue around wound are effectively described.
Pressure Ulcer or incision Documents location, appearance, stage, drainage and if a dressing is present or changed.	Poor Evidence that minimal effort made to describe wound appearance. Few specifics included. Two or more missing elements of documentation.	Fair Evidence that 4/5 elements of appearance are included in charting.	Good Evidence that all five elements of wound or incision appearance are included.
Wound measurements Includes wound dimensions in appropriate units, mm for tiny wounds, cm for larger, inches when policy dictates.	Poor Incomplete measurements of wound or none.	Fair Missing either length, width or depth measurements or uses incorrect units to describe wound size.	Good Dimensions of wound and surrounding involved tissue are effectively described and include width, depth and length in appropriate units for wound/incision size.
