Effort/Cooperation	Poor Never showed effort and/or cooperation with classmates and teacher. Never followed the rules or directions.	Fair Sometimes showed effort and cooperation with classmates. Only followed the rules when it was convenient.	Good Often put forth a good effort and was cooperative with classmates. Followed the rules most of the time.	Excellent Always put forth best effort and fully cooperated with classmates. Was a self-starter and followed the rules.
Game Play	Poor Never participated or seemed interested in the games.	Fair Sometimes participated and seemed slightly interested in the games.	Good Enjoyed the games and participated most of the time.	Excellent Enjoyed the games, participated all the time, and had fun with the rule changes.
Skill Warm-up	Poor Never put forth an effort during the warmup.	Fair Sometimes did the warm up but not with much effort.	Good Often did the warmup and put in enough effort. Enjoyed movement related to fitness.	Excellent Always put in a lot of effort in the warmup and really enjoyed movement related to fitness.
