I worked PRODUCTIVELY in my team work. I rarely left my seat, I stayed focused while in groups.
Good

I was MOSTLY on-task and worked productively today. I left my seat once and stayed mostly on task while in groups.
Fair

I was MOSTLY off-task and UNPRODUCTIVE today. I left my seat frequently and needed to be reminded to sit down. I needed to be refocused during group work.
Lack effort

I was NOT on-task and UNPRODUCTIVE, AND/OR I made excessive missing from group work, I rarely participated in group work.

I was PREPARED for group work with ALL materials, content and resources needed to complete assignments.

I FREQUENTLY displayed disruptive behavior such as: speaking out of turn, interrupting the teacher, derailing conversations, and distracting my classmates. I frequently needed to be redirected by my classmates and/or teacher.

I displayed EXCESSIVE disruptive behavior such as: speaking out of turn, interrupting the teacher, derailing conversations, and distracting classmates. My classmates and/or my teacher had to remind me many times during class today.
