Catching

Level 1 Poor

Cannot track the object.
Level 2 Fair

Eyes closed or turn head away from ball.
Does not move to catch ball.
Catches ball with straight arms away from body or traps ball between body and arms.
Level 3 Good

Eyes on ball but may flinch.
Reaches to catch ball.
Catches with hands only.
Level 4 Excellent

Eyes on ball.
Moves to catch ball.
Catches with hands onlly and brings ball in to chest.
Underhand Throw

Level 1 Poor

No step.
No backswing.
Untimely object release.
Level 2 Fair

Steps forward with wrong foot.
Backswing with no follow through.
Inconsistant object release
Level 3 Good

Steps forward with correct foot.
Full backswing and follow through.
Coordinated arm motion with smooth release.
Level 4 Excellent

Throws with proper form and also demonstrates power and consistant accuracy.
Overhand Throw

Level 1 Poor

Faces Target.
No step.
No trunk rotation.
Lack of accuracy.
Level 2 Fair

Slight front to back stance.
Steps with wrong foot.
Minimal trunk rotation or bends forward.
Inconsistant accuracy.
Level 3 Good

Stands with side to target.
Steps with correct foot.
Consistant trunk rotation.
Level 4 Excellent

Throws with proper form and also demonstrates power and consistant accuracy.
Kicking From a stationary position

Level 1 Poor

No steps are taken to approach the ball(stand and swing) contact is made with the toe, no follow through. Shot is erratic and not accurate.
Level 2 Fair

Steps are taken to approach the ball but steps may not be balanced. Plant foot is near ball but not next to ball. Contacts the ball with toe or side of the foot. Little or no follow through resulting in loss of power.
Level 3 Good

Students leg is bent at the knee and arm swing helps with the distance of the kick. Plant foot is around the ball but not in proper spot. Contacts the ball with instep. Average contact with ball, eyes on target.
Level 4 Excellent

Student's leg is bent at the knee before kick. Correct arm action helps with the force of the kick. Plant foot is correct. Follow through correct, hits with instep, good contact with ball eye on target.
Baseball Hitting

Level 1 Poor

Feet stationary, whole body turns with swing, hands spread apart or reversed. Eyes not on ball or closed while swinging.
Level 2 Fair

little foot movement, partial body turn during swing, hands together not lined up right, eyes not following ball.
Level 3 Good

Front foot moving towards pitch, back foot pivots not leaving ground. body stays closed shoulders do not open up and face pitcher. Hands are correct, lined up and wrist rolls over with swing. Eyes follow ball all the way.
Level 4 Excellent

Front foot moving towards pitch, back foot pivots in bug squashing motion never leaving the ground. Body stays closed, shoulders do not open and face the pitcher. Hands gripping bat correctly, knuckles lined up, wrist roll over through the swing, eye follows ball throughout the swing, good fluid swing, solid contact and follow through.
Striking with a hockey stick

Level 1 Poor

Only one hand. or hands at top close together, feet close together, poor balance. Brings stick too high, often misses or barely hits puck.
Level 2 Fair

2 hands on stick but awkward, feet apart but no weight shift.Brings stick below waist but ends below ankles makes contact most of the time, sometimes making a good strike.
Level 3 Good

Bring stick back just below waist and ends swing between knees and waist. Makes contact with puck most the time, puck travels with spinning motions and good velocity.
Level 4 Excellent

Consistent good swing, follow through and contact with the puck, puck travels with spinning disc motion and good velocity.
Sportsmanship

Level 1 Poor

Student cannot list any attributes of good sportsmanship
Level 2 Fair

Student demonstrates one aspect of good sportsmanship (safety rules, treating others with respect and appreciation)
Level 3 Good

Student demonstrates both aspects of good sportsmanship with little prompting.
Level 4 Excellent

Student shows both aspects of good sportsmanship throughout and after activity.
