Wound cleansing

Gathering supplies	Poor Student does not gather any supplies.	Fair Student leaves the patient multiple times to gather various supplies.	Good Student gathers supplies prior to procedure but forgets 3-5 items and retrieves them in one trip.	Great Student gathers most supplies with the exception of 1-2 items and retrieves items in one trip.	Exceptional Student gathers appropriate supplies including: clean gloves, sterile normal saline, chux, gauze dressing, tape, waste receptacle, clean sheet, gown, gloves, and mask with eye shield.
Preparation	Poor Student removes dressing without assessing patient, donning PPE, or setting up work area.	Fair Student dons PPE but does not assess patient's pain or prepare work area.	Good Student dons PPE and assesses pain but does not prepare work area.	Great Student dons PPE and assesses pain. Student prepares work area but does not provide privacy for the patient and/or forgets items in work area.	Exceptional Student assesses patient for pain management, places the patient in a comfortable position, places the chux under the patient and uses the sheet to expose only the area of the wound. Patient sets up supplies near the patient on a clean field and dons appropriate PPE.
Assessment	Poor Student performs no assessment of the wound prior to beginning next step.	Fair Student carelessly removes dressing, performs only a partial assessment, and does not remove gloves before beginning next step.	Good Student carelessly removes dressing, performs an assessment forgetting some detail, and does not remove soiled gloves prior to beginning next step.	Great Student carefully removes dressing, performs assessment forgetting only minimal detail, and removes soiled gloves.	Exceptional Student carefully removes old dressing with adhesive remover and saline PRN. Student assesses wound dressing for presence of, amount, odor, and color of drainage. Student assesses wound color, size, tunneling, undermining and peri-wound. Student removes and disposes of soiled gloves.
Irrigation & dry dressing	Poor Student does not perform hand hygiene, put on clean gloves, or cleanse wound prior to applying dry dressing.	Fair Student does not perform hand hygiene or don clean gloves prior to cleansing wound and applying dry dressing.	Good Student performs hand hygiene and dons clean gloves prior to cleansing but applies some other dressing than a dry gauze dressing.	Great Student performs hand hygiene, dons clean gloves, but forgets to irrigate wound prior to applying dry-gauze dressing.	Exceptional Student performs hand hygiene, dons new clean gloves, irrigates wound with saline bullet and sterile gauze. Student gently dries wound with new sterile gauze and applies new sterile gauze dry dressing and tape.
Documentation	Poor Student does not document wound cleansing and dressing.	Fair Student documents that they performed wound cleansing and dressing but does not document wound assessment.	Good Student documents wound cleansing, dressing, and a partial wound assessment.	Great Student documents wound cleansing, dressing, and wound assessment but forgets minor details in wound assessment.	Exceptional Student documents wound cleansing, dressing, and complete wound assessment.
