Wrap a sterile Pack	Pass Student placed items in the pack and wrapped securely with 2 layers of linen. Tabs were correct, pack was tight enough to not fall apart but loose enough to allow sterilization to take place, indicator strip was placed in the middle of the pack, indicator tape was placed and labeled correctly	Fail Student failed to do one or more of the requirements of the skill.
Unwrap a sterile Pack	Pass Student unwrapped the first layer without contaminating the inner layer of the pack on a clean, dry, level surface.	Fail Student breached sterility
Gown	Pass Student unwrapped the gown and did not breach sterility, placed the gown on without breaching sterility and waited for someone to tie the back of the gown so they could perform the hand scrub.	Fail Student breached sterility.
Perform Sterile Hand Scrub	Pass Student washed hands first, cleaned fingernails, started scrubbing at fingertips and advanced to elbows. Hands were always above elbows, sterilization was not breached. Jewelry, fingernail polish and fake nails were removed before scrub.	Fail Student breached asepsis.
Glove	Pass Student did not breach sterility with the gloves.	Fail Student breached sterility.
Run Autoclave	Pass Student correctly loaded or made sure it was loaded correctly and ran the correct cycle.	Fail Student missed one or more of the steps.
Leak Test Machine	Pass Student turned on oxygen, occluded the breathing tubes, closed pop off valve and allowed the pressure to get to 20mmHg, holding the pressure there until it was verified there was no leak. Pop off valve was open and oxygen was turned off.	Fail Student did not successfully complete the leak test.
Complete Controlled Drug Log	Pass Student completed the drug log.	Fail Student did not successfully complete the controlled drug log.
Calculate Drug Dosages	Pass Student successfully completed the drug dosages in mg's and ml's.	Fail Student did not successfully calculate the correct dosages in mg's and ml's.
