Journal Assessment
	Poor 1 pts	Fair 2 pts	Good 3 pts
Organization	Poor Incomplete set up, loss of format, and has many titles or examples not completed. Very hard to read due to handwriting, erasures, or messy.	Fair Set up is is fairly complete and has a 2 page format, with most data, titles, & examples. Handwriting is somewhat clear. Missing one section.	Good Journal is set up correctly and includes TOC, 2 page format, including titles, graphics, & examples with necessary data. Is legible, neat, and complete.
Teacher Data Page	Poor Most data is not copied or written so that there is little understanding of concept by student.	Fair Some data is copied, but is difficult to understand due to incomplete data not written by the student.	Good All data has been copied that is necessary for concept understanding and included by the student.
Student Data Page	Poor Little to no response is evident. Grammer and convetions poor. Many mistakes in spelling or fragmented thoughts.	Fair 4 out of 5 responses have been addressed. Most grammer and conventions are clearly evident.	Good Student has addressed responses to: Topic, I Can statement, What I Knew, What I Learned, Proof, and Reflection. Written clearly and has good conventions
Proof	Poor Has incorrectly displayed an example or not completed.	Fair Has displayed examples, solutions, or written algorithym. Shows some understanding of concept.	Good Has shown creativity through examples, color, written expressions or pictures, & graphic aides demonstrate understanding of concept presented.
Reflection	Poor One or less sentences to explain the necessity of this concept and its usefullness to the student.	Fair Written twp or three sentences that explains why this concept is useful to this students' math career.	Good Written 4 or more sentences that show understanding of why concept may be important, useful, or necessary for success in math, jobs, or future endeavors.
