(-) Effort Diagnosis portion is incomplete or irrelevant.
Information Sheet 33 % Organization, Creativity, Engaging	Excellent -The information sheet is organized and is not just a list of facts. -The information sheet is presented in a creative and concise manner with full understanding of the disease process.	Good -The information sheet is clear and logical and contains facts as well as very few mistakes. -Good clear presentation.	Fair -Some mistakes are made. -Little organization and clarity is used. -Lacks creativity -There is little style and reads more like a list of facts rather than a patient education sheet.
