Choral Audition Form
	Poor 1 pts	Good 2 pts	Excellent 3 pts
Pitch Accuracy--echoing portion	Poor The student cannot match most pitches, and has difficulty hearing notes. Lacks confidence.	Good The student can accurately match more than 50 percent of pitches. Has a fair amount of confidence.	Excellent The student can accurately match at least 90 percent of pitches. Student performs pitches confidently.
Rhythm Accuracy--echoing portion	Poor The student cannot match rhythms and lacks confidence.	Good The student can accurately match more than 50% of given rhythms: Student has a fair amount of confidence.	Excellent The student can accurately match at least 90% of rhythms. Student performs rhythms confidently.
You Raise Me Up	Poor Student could not sing the song on the pitch given. Student changed pitches and had trouble singing the melody of the song.	Good Student sang the song correctly. There were a few pitch problems, but the melody of the song was more than 75% accurate. OR Student did not start on the correct pitch; but was able to finish the song correctly after being redirected.	Excellent Student sang the song with a high degree of accuracy!
Behavior/Attitude	Poor Student has poor attitude. Shows negativity.	Good Student shows a mostly positive attitude. Portrays some leadership skills.	Excellent Student displays a positive attitude. The student shows potential of being a great leader and works very well with others.
