Social Skills & Participation Data to be taken 1x per week (on Thursday or Friday) in the General Education setting to rate Pedro's classroom and social performance for the entire week.
	Poor 1 pts	Fair 2 pts	Good 3 pts	Excellent 4 pts
Problem Solving & Managing Material	Poor Pedro does not respond when asked to get materials and does not begin working without multiple physical, verbal, or gestural prompts from teacher or peers.	Fair Pedro rises to collect his needed materials but does not know where to get them and does not ask for help. May require multiple verbal prompts to begin working once he is seated.	Good Pedro may require 1 additional verbal prompt to locate materials or asks for help from others. Begins working once seated but may require 1-2 prompts to complete the activity.	Excellent Pedro is able to get his materials independently and begin his work upon request.
Peer Interaction & Play	Poor Pedro does not interact with his peers.	Fair Pedro will enter into play activities with peers with multiple verbal, physical, gestural directives from teacher or peers and requires multiple prompts and reminders to maintain interaction.	Good Pedro may enter into play with peers when directed but requires 2-3 prompts to maintain interaction and play.	Excellent Pedro interacts with peers appropriately in play with no more than one verbal prompt. (participates, shares, follows directions, takes turns).
