Understands parts of simple instructions, but has some confusion with others. Will attempt multiple times, but ask for help when needed. Has a grasp of most simple shapes, objects, letters, and number but has difficulty with certain ones.

Has a strong comprehension of simple instructions. Will make multiple attempts until the task is completed, but ask for help when needed. Has a strong grasp of simple shapes, objects, letters, and numbers.
Social Development

Has a difficult time playing with others. Gets angry at other children easily. Cannot resolve any issues by themselves. Plays by themselves, distances themselves from others.

Makes attempts play with others. Plays well with a few. Tries to resolve issues, but needs assistance from an adult.
Good
Plays well with most others. Tries to resolve most issues, until it is resolved. Asks for help from an adult, only when necessary.
Gross Motor Development

Has difficult with most simple large motion involved tasks. Such as: difficult standing on one foot for less than 5 seconds, galloping, jumping 2-4 feet, throwing a ball upper and under hand, kicking a ball, etc.
Fair
Can manage a fair amount of simple large motion involved tasks, but has difficulty with some.
Good
Can manage most to all simple large motion involved tasks.
Fine Motor Development
Poor
Has difficulty with simple self-help skills: such as combing hair and washing hands. Cannot put shoes on correct feet, or zip and unzip jacket and pant zippers. Has difficulty using simple eating utensils.
Fair
Can perform simple self-help skills with small amounts of help from adults. Can put shoes on right feet sometimes, but other times needs help. Can zip zippers with little adult help. Can use simple eating utensils with little adult help.
Good
Can perform simple self-help skills independently. Can out shoes on correct feet independently. Can zip zippers independently. Can use simple eating utensils independently.
Creativity Development
Poor
Has trouble drawing and creating simple shapes, like circles and lines. May struggle coming up with an idea to create, and need a lot of adult assistance.
Fair
Can create a few simple shapes, such as circle and line, but needs assistance. Needs small amounts of adult help with making up an idea.
Good
Can easily create and draw simple shapes. Easily creates an idea. May name their creation.
Life Skills
Poor
Needs adult assistance with most to all tasks and situations.
Fair
Only asks for adult assistance with some tasks and situations.
Good
Asks for little to no help from adults with most tasks and situations. Wants to be very independent.
