	Poor 5 pts	Fair 15 pts	Excellent 20 pts
Department Audit 30 %	Poor The departmental audit is turned in late, and is missing key parts, or a majority of information is incorrect.	Fair The departmental audit is turned in, but is missing information, or is not timely.	Excellent Information is collected in a timely fashion, and is thoroughly researched and correct.
Chart Audits 10 %	Poor All 5 of the chart audits are not completed.	Fair All 5 chart audits are turned in, but some information is missing or incorrect.	Excellent All 5 chart audits are completed in a timely fashion, and all information is correct.
Group Facilitation Audits 10 %	Poor All 5 group facilitation audits are not completed.	Fair All 5 group facilitation audits are completed, but some suggestions and information represented on the audit are incorrect.	Excellent All 5 group facilitation audits are completed in a timely fashion, and suggestions and information represented on the audit is well thought out.
QI Project Report 30 %	Poor The QI report is not completed, or is completed but needs major revisions.	Fair The QI project report is completed, but revisions are needed.	Excellent The QI project report is well-written, and reflects a current need of the department, and how the need was addressed over the past quarter.
Presentation of Finding 20 %	Poor The presentation of the material is unorganized and unclear.	Fair The presentation of the material needed better organization or clarity.	Excellent The student presents information obtained from audits in a well-organized and clear manner.
