Preparedness	Unsatisfactory Student rarely brings his/her required materials and completed assignments. Student has to borrow supplies three or more times.	Needs Improvement Student sometimes brings his/her required materials and completed assignments. Student has to borrow supplies no more than twice a week.	Good Student usually brings required materials and completed assignments. Student may need to borrow supplies once.	Excellent Student always brings required materials and completed assignments to class. Student doesn't need to borrow any required supplies.
Active Involvement	Unsatisfactory Student rarely listens to the instructor, disrupts class and is not involved with the class activity.	Needs Improvement Student sometimes listens to the instructor, occasionally disrupts class and needs a reminder to get on task.
