Good At least 6 of the questions are addressed. The community is primarily described.	Fair At least 4 issues are addressed, a clear picture of the community is not accomplished.	Poor Less than 4 issues addressed, vague and non-descriptive.
Social Characteristics	Excellent All census data is provided for each category with references.	Good At least 7 sections are described.	Fair At least 5 sections are completed.	Poor Less than 5 sections or inaccurate information.
Issue of Concern	Excellent Problem is clearly identified with the etioloty and proof of concern among community members. Intervention is based on research and understanding of community.	Good Problem identified, but not in detail, intervention is too general.	Fair Problem is slightly identified, intervention not clearly articulated.	Poor Vague problem identifcation, no intervention plan provided.
References	Excellent Use of at least 15 references, all within the last 2 years and correctly formatted APA style.	Good Between 11-14 references.	Fair Between 7-10 references.	Poor Less than 7 references, APA format not followed.
