Change for challenging behaviors 
This rubric scores the participant on number of behavioral episodes, the intensity of behavioral episodes and the ability to self regulate during situations. It is scored so that the higher the percentage, the more frequent, intense the behavior, and the less self-regulation used. Goal is to decrease the challenging behavior.

Exhibits the next behavior within the chain of the episode (e.g. staring "through" someone)
Fair

Exhibits the next behavior within the chain of the episodes (e.g., physically approaching another person)
Poor

Full episode and may include becoming physically aggressive towards others
Self-regulation

Excellent

Uses deep breathing techniques to calm himself before he becomes physically aggressive, unprompted 0-1 times per week.
Good

Uses deep breathing techniques to calm himself with prompting 2-3 times per week before becoming physically aggressive.
Improving

Goes to his quiet place to calm down without prompting 4-5 times per week.
Fair

Goes to his quiet place with verbal prompting 1 time per day.
Poor

Requires external cues (e.g., physical and verbal prompts) more than one time a day to go to his quiet place to calm down.
