Pulse	Exemplary Found the pulse quickly at the wrist and took it correct the first attempt.	Proficient Found the pulse at the writst after several minutes.	Needs Practice Had difficulty finding the pulse at the wrist, but fianlly was able to get this correct.
Respiratory rate	Exemplary Able to count on first attempt without cues from instructor and record correct respirtory rate.	Proficient Able to count on first attempt but required cues from instructor on how to get a correct respiratory rate.	Needs Practice Was not able to count rate on first attempt and needed to repeat more than once to get correct respiratory rate and needed cues from instructor.
Explain procedure to client	Exemplary Explained to client what you will be doing and asked if there is any problem. Also asked what their normal blood pressure is.	Proficient Explained to client but forgot to ask what normal pressure is.	Needs Practice Forgot to ask if there are any problems and what the normal is, but did explain the procedure.
Apply blood pressure cuff	Exemplary Correct size BP cuff selected. Palpate the pulse point. Apply BP cuff with arrow mark above artery, gauge visiable. Apply cuff evenly and snuggly around extremity.	Proficient Did some fumbling with the cuff, but got it on correct.	Needs Practice Needed several tries to do this and had to adjust the tightness.
Took blood pressure	Exemplary Handled the gauges corrected and inflated the cuff just above where it needed to be. Let the air out slowly but not painfully slow.	Proficient Did correct but took several attempts to get the inflation knob correct.	Needs Practice Needed several attempts to get the cuff inflated and left it up too long causing some pain.
Obtained reading	Exemplary Obtained the correct reading on the first try.	Proficient Obtained the correct reading on the second try.	Needs Practice Obtained the correct reading on the second try but demonstrated little confidence.
