Health Nutrition Paper
Each student will write a (3-5 double-spaced pages) research paper on a topic concerning a health issue related to nutrition (e.g., diabetes, obesity, beriberi, etc). This must be an approve topic beforehand. Your paper needs to include a references citing at least 5 sources in MLA or APA Format. Paper is due on Monday, Nov. 16 (5 points will be taken off for everyday it will late *weekend counts**)

MyPlate Assessment Rubric
1. Students will use a nine-inch paper plate to create their own MyPlate. 2. Color plate to resemble the colors on MyPlate. 3. Label each food group and cut or draw foods (at least two foods per group) that represents MyPlate food groups.

Writing Prompts Rubric
Students will use this rubric for their daily in class writing prompts. Writing prompts will be graded each month with 15 prompts per a month. Students can make up prompts for days they are absent, but it will be their responsibility to learn of prompts they have missed and they will be expected to write those prompts on their own time. A total of 25 points given for this assignment. 15 points for completion, which is 1 pt per prompt finished. 10 points for overall writing standard.

One-Week Meal Plan
Students will create a healthy meal plan for Five days including breakfast, lunch and snack (mini-meal) for kids attending after-school program. Also included should be healthy daily caloric intake, a proper portion control for each meal and a balance of learned food group.

Wellness Wheel Rubric
The poster will contain information directly related to the theme of The 6 dimensions of wellness. The poster must include a title and the 6 dimensions of wellness (physical, intellectual/mental, emotional, social, spiritual and environmental). Students will create a poster of their strengths within each dimension of wellness. The poster will be designed on poster board using magazine/newspaper cut outs, or clip art/other computer drawings.
